Job Description:
Trinity Health At Home is seeking a dedicated and compassionate
Registered Nurse (RN) to join our team providing high-quality
nursing care to patients in their homes. This role is pivotal in
helping patients achieve recovery and manage chronic conditions by
delivering personalized care plans, administering medication, and
monitoring patient health. The ideal candidate will have experience
in home health care, a patient-first approach, and a commitment to
fostering a supportive environment for patients and their families.
Assess patient health problems and needs, and develop and implement
nursing care plans Maintain accurate, detailed reports and records
of patient care Administer medications and treatments to patients
and monitor patients' conditions and responses Instruct patients
and their families on how to manage illnesses or injuries and
post-treatment home care needs Consult and coordinate with
healthcare team members to assess, plan, implement, or evaluate
patient care plans Strong interpersonal and communication skills
Proficient in maintaining accurate and detailed records Expertise
in the administration of medications and treatment Ability to
instruct patients and families on health care essentials Adept in
utilizing various medical equipment and technology Medical
Insurance Dental Insurance Vision Insurance Life Insurance 401k
Valid Registered Nurse (RN) license CPR certified Possession of or
ability to obtain a valid driver's license (as travelling to
patient homes is required)
