Job Description Job Description Harbor is seeking a nurse with
experience working with mental health and/or substance abuse
clients of all ages. LPN or RN required. Position is full-time, 40
hours per week. Education/Experience/Other Requirements : Current
Licensed Practical Nurse (LPN/LVN) license in good standing
required, or Associate’s degree from an accredited school of
Nursing with a current R.N. license in Ohio required. 3 years of
experience working in a psychiatric nursing/SUD setting preferred.
Must be proficient and accurate in computer use, including
Microsoft Word. CPR/BLS and NCI/CPI Certifications required within
90 days of employment. Must have a valid driver’s license,
acceptable driving record, and show proof of personal auto
insurance policy to be deemed insurable with Harbor’s vehicle
insurance carrier. Experience in behavioral health, addiction
medicine, or related healthcare setting preferred. Knowledge of
substance use disorders, withdrawal management, and
medication-assisted treatment. Strong crisis management,
assessment, and communication skills. Ability to work
collaboratively in a multidisciplinary team. Certification in
addictions nursing (CARN or CARN-AP) preferred. Essential Job
Competencies/Primary Duties: Responsibility for the site includes
the following: reviews and updates treatment plans for clients
coordinating insurance forms and pre-certifications as necessary;
enters prescribed medication into medication module; Conduct
initial and ongoing nursing assessments, including physical, mental
health, and substance use histories. completes quarterly checks of
first aid boxes, bloodborne pathogen kits and maintenance of
medical equipment; amount of time necessary to manage a site will
be determined by supervisor and the nurse managing the site.
Provides medication management of clients through: dispensing
medications per medical providers' order Collaborating with other
medical and clinical team members. Responsible for managing
medication rooms, maintaining records of medication use and
disposing of expired medications. When necessary, assists other
clinical staff with crisis intervention and pre-hospital screening.
Provides referral/linkage of client to appropriate resources.
Completes the required number of peer review forms. Ensure accurate
documentation of patient care, interventions, and progress in the
medical record. Conduct initial and ongoing nursing assessments,
including physical, mental health, and substance use histories.
Monitor patients for withdrawal symptoms, administer medications as
prescribed, and utilize standardized withdrawal assessment tools
(e.g., CIWA, COWS). Provide support to providers in reviewing and
initiating labs and coordinates patient medical paperwork requests.
Attends meetings as required and serves on committees as requested.
Participates in professional meetings, workshops, seminars, and
other continuing education programs to keep current with
professional practice, to contribute to his/her professional growth
and to maintain licensure. Participate in discharge planning by
providing referrals, resources, and patient teaching to support
continuity of care. Will facilitate groups 1-2 times per week or as
decided by the medical team. May be required to work up to eight
(8) hours of on-call duty per week, which may include evenings,
