This role involves providing exceptional end-of-life care to patients and supporting their families with empathy and professionalism. The ideal candidate will have a strong background in hospice nursing and be committed to delivering patient-centered care in a home setting. Responsibilities Provide skilled nursing care to hospice patients in their homes, ensuring comfort and dignity. Assess patient needs and develop individualized care plans in collaboration with the interdisciplinary team. Monitor patient condition, document observations, and report changes accurately to the care team. Educate patients and families about hospice care, symptom management, and medications. Collaborate with physicians, social workers, and other healthcare professionals to coordinate care. Assist with pain and symptom management, including medication administration as prescribed. Maintain compliance with all regulatory and organizational policies and standards. Requirements Valid Registered Nurse (RN) license in the state of Michigan. Previous hospice or palliative care nursing experience preferred. Strong clinical assessment and patient care skills. Ability to work independently and manage time effectively in a home care setting. Excellent communication and interpersonal skills. Current CPR certification. Reliable transportation to visit patients at their homes in Livonia, MI.
