Job Description Job Description In accordance with the Agency’s
mission statement, policies and procedures, professional Code of
Ethics, State of Michigan licensing, contracts, funding, and COA
standards; the S.T.E.P.S. Program Supervisor is responsible for
being a liaison with funders and ensures fidelity to grant and
contract expectations while assisting in the implementation and
evaluation of all S.T.E.P.S. Student Programming in partnership
with the S.T.E.P.S. team. This position will provide services such
as assessment, service planning, and advocacy, linking,
coordinating, and teaching life skills, to assist families in
attaining an independent and self-sufficient lifestyle. TASKS OF
THE POSITION • Monitors quality individual, group, and/or family
counseling according to specific treatment plans. • Serves as a
liaison and client advocate with the court, funding,
referring/referral agencies and schools. • Monitors all required
paperwork, (e.g. updated assessments/treatments plans, case notes,
phone logs, court reports, Child and Adolescent Functional
Assessment Scale (CAFAS) and utilization review reports). •
Schedules, organizes and attends team decision meetings. •
Maintains flexible work schedules that provide accessibility to
support the needs of the family. • Provides education and support,
regarding needs of clients, and families. • Participate in
continuous quality improvement efforts. • Stays informed of current
practice developments within the profession and uses educational
and training opportunities to ensure continual professional
development and competence. • Plans and organizes work to meet all
requirements according to agency and COA standards, licensing and
funding requirements. • Prepares for and participates in individual
supervision, team meetings, and other agency meetings. • Works with
Vista Maria programs and community agencies/resources to provide
referrals, resources and related services to youth. • Participates
in weekly contact with the child and family and/or members of the
Child and Family Team regarding the implementation of the
wraparound plan. • Ensures assembly of a Child and Family Team
within two (2) weeks of enrollment by interviewing the family,
identifying family members/natural support/agency representatives
and other significant people. • Assemble the Child and Family Team
and guides members throughout the wraparound process, ensuring
adherence to the model fidelity and wraparound values. •
Responsible for assisting each youth and family in defining the
frequency of child and family team meetings, the standard for
meeting frequency should occur at least weekly until the family
begins to transition toward program completion. Once the
transition/completion plan is developed, the meetings should then
occur at least bi-weekly for no longer than 3 months. • Works with
the family to uncover youths/family’s strengths and needs, aids
with any immediate needs, and complete the initial strengths
assessment, needs, and priorities within (30) days of enrollment. •
Work with the families and submit a comprehensive support plan
(proactive and reactive crisis/safety plan) within 1 week of
enrollment. The plan must reflect the best possible fit with
strengths, culture, family, and beliefs of family. • Work with the
Child and Family Team and submit an Individualized Plan of Care
(POC) with services and resources that are community based and
culturally relevant within 30 - 45 days of enrollment and an
updated plan every 90 days thereafter. • Ensures the needs and
services identified by the child and family team are met. • Ensure
community resources are sought first with the assistance of the
child and family team and the community Team. • Collaborates with
other necessary individuals the youth and family has contact with,
such as, DHHS/CMO Workers, Teachers, Judges, District Attorneys,
Mental Health Clinicians, Physicians, etc. • Maintain frequent
contact and invite them to child and family team meetings with
adequate notice and provide copies of the plan of care within 1
week of completion for team members. • Ensures all barriers related
to Plan implementation are addressed. • Facilitate necessary
referrals, information and cooperation with collateral resources to
ensure quality care and service delivery. • Monitor the provision
and quality of services provided to the family and is the liaison
when new services/resources need to be sought or developed. •
Complete necessary paperwork in a strength-based manner. • Assist
in the development, implementation and evaluation of all S.T.E.P.S.
Programming in partnership with the S.T.E.P.S. leadership team. •
Evaluate the Youth Assistance Program for level of youth
participation; to include valuable youth input on current and
future programs offered. • Assist in developing and implementing
marketing efforts for Youth Assistance programs to students,
families, and potential donors and volunteers. • Acts as the agency
liaison for school and community partners in community programming,
monitoring programming, service elements, participation and
ensuring off campus objectives and goals are achieved. • Works with
staff to develop team and individual goals to achieve agency and
contract objectives including administering staff performance
evaluations. • Trains staff to ensure services are delivered to
students in accordance with the Vista Maria core values and that
documentation and program criteria and plans are met. • Assists in
the management of operational direction of staff and all components
of after school programs. • Maintain necessary tools for tracking,
reporting, scheduling, and processing systems. • Attend training
sessions or professional meetings to develop or maintain
professional knowledge. • Monitor case notes, case files, and
lesion plans. • Monitor the results and plans from pre and post
surveys for program improvement. • Work with other supervisors to
ensure that the agency staff are referring JJ youth to the
appropriate JTS services. • Monitor compliance with COA
requirements. • Performs all other duties as required or requested
by the agency. • Demonstrated leadership capacity and understanding
of teambuilding skills. • Demonstrated proficiency in basic
computer skills. • Demonstrated ability to interact positively in a
culturally diverse environment. • Knowledge of tracking and reports
tools used to support treatment services. • Ability to work a
flexible schedule including evenings and weekends as needed. •
Working knowledge of diagnostic criteria and coding. (CANS, FANS).
SUPERVISORY RESPONSIBILITY S.T.E.P.S. Family Services Specialist,
S.T.E.P.S. After School Activities Coordinator, S.T.E.P.S. Dream
Mentor Coordinator, and S.T.E.P.S. Workforce Development
Specialist. Minimum Required: • Bachelor’s degree in the Human
Services field, from an accredited school, in the areas of Social
Work, Psychology, Sociology, Criminal Justice and Guidance
Counseling. • Two (2) years of supervisory experience in any child
welfare component. • Three (3) years of experience in a residential
child caring facility or a juvenile justice, child welfare. • Valid
driver’s license without relevant restrictions. • Valid vehicle
insurance and appropriate registration. Preferred: • Master’s
degree in the Human Services field, from an accredited school, in
the areas of Social Work, Psychology, Sociology, Criminal Justice
and Guidance Counseling. • Completion of Child Welfare Institute
(CWI) training required by the State of Michigan. • Experience with
at risk youth and families. • Experience with after school and
community-based teen programs. • Experience in child caring
institutions or child placing agency. • Educational administrative
experience in a high school setting. • Knowledge of tracking and
reporting tools used to support treatment services. • 1-2 years
same or similar experience (i.e. Independent Living Youth) • Child
and Adolescent Functional Assessment Scale (CAFAS) trained. •
Experience with grant writing and fundraising activities. • Fully
